The Charity Banking Survey reveals what banks are most popular with charities and how charities rate their performance.
It also canvasses views on ethical banking and loan finance.
All respondents receive a copy of the final report, worth £109, as well as being entered into the prize draw.
The survey is confidential, so respondents can answer candidly and in confidence. The deadline for completing the questionnaire is this Thursday.
